OneCare Kansas services available April 1

The Center for Counseling & Consultation will become a provider for OneCare Kansas beginning April 1. OneCare is a program of KanCare, Kansas Medicaid. It does not change a person’s KanCare benefits and is provided at no additional cost, said Julie Kramp, executive director at The Center.

Often people who have complicated health and behavioral conditions feel frustrated and overwhelmed working with multiple clinics, specialists, insurance companies and government agencies, Kramp explained. As a OneCare Kansas provider, The Center will offer comprehensive health care coordination, health education and promotion, coordination of transitional care from inpatient to other settings, appropriate follow-up, support for family members who assist, and referral to additional medical and social services.

A Care Coordinator will help each OneCare member develop a personalized Health Action Plan. The Care Coordinator then stays in touch with the OneCare member and his or her medical and related providers to make sure everyone understands what is needed for this person to stay as healthy as possible.

Initially, OneCare Kansas will be available to people who are diagnosed with asthma, paranoid schizophrenia, severe bipolar disorder, and other risk factors. The Center encourages those who feels they may qualify to visit www.kancare.ks.gov and select “Member Information,” or simply to call The Center at 620-792-2544 and ask to speak to a OneCare Coordinator.
