Carol Ann Cook, 71, died Sept. 11, at home in Great Bend. She was born March 1, 1943, at Kinsley, to Bernard and Daisy (Goad) Fahlsing. She married Charles Banks in 1961. She later married Phillip Ward in 1976. She then married Dennis Cook on Aug. 11, 2003, at Las Vegas, Nevada. She worked in retail for Revco and Dillons until retiring.
Mrs. Cook was a Great Bend resident since 1976, coming from Carrollton, Texas.
She was a member of First United Methodist Church, Great Bend; and Veterans of Foreign Wars Post 7428 Auxiliary, Hoisington.
Survivors include her husband, Dennis Cook, of the home; one son, Mark Banks of Great Bend; one stepson, Richard Ward of Great Bend; one daughter-in-law, Jacqueline Carey of Great Bend; three brothers, Eddie Smith of Surprise, Ariz., Doug Smith of Boulder, Colo., and Darrell Smith of Grovetown, Ga.; two sisters, Betty Palmer of Glenwood, Iowa, and Sue Jaske of Great Bend; nine grandchildren, including Heather Madrid of Great Bend, who she raised; and three great-grandchildren. She was preceded in death by a stepson, Shawn Cook; daughter, Cara Stevens; and two brothers, Bobby Fahlsing and Ronnie Smith.
Memorial service will be at 1:30 p.m. Tuesday, Sept. 16, at Bryant Funeral Home with the Reverend Lennie Maxwell, officiant. Inurnment will be at Hillside Cemetery, Kinsley, at a later date. The family will receive friends from 6-8 p.m., Monday at the funeral home. Memorial fund donations to First United Methodist Church may be made in care of Bryant Funeral Home.
