CHASE — Heather D. Radcliffe, 38, died Jan. 9, 2015, at her residence in Chase.
She was born Dec. 9, 1976, at Lyons, the daughter of Ronald D. and Judy Lee Gates Wilcox. She graduated from Chase High School in 1995, and attended Hutchinson Community  College. She married Derek Radcliffe on June 21, 1997, at Chase. A lifetime Chase resident, she was a homemaker, CNA and worked in Medical Records at Lyons Good Samaritan Center, Lyons.
Mrs. Radcliffe was a member of First Baptist Church, Chase.
Survivors include her husband Derek, of the home; two sons, Ryan and Nathan Radcliffe, of the home;  mother, Judy L. Wilcox, Chase; two sisters, Lori Ann and Jeff Wisehart, Russell, and Lisa Kay Havard, Chase; four nieces and one nephew.
She was preceded in death by her father, Ronald Wilcox; and grandparents, Junior and Goldie Gates, Roy D. and Ellen Wilcox.
Funeral service will be at 10:30 a.m. Monday, Jan. 12, at First Baptist Church, Chase, with the Rev. Charles Kirk officiant. Burial will be at Lyons Municipal Cemetery. Visitation is scheduled from 2-6 p.m. Sunday, with family present from 4-6 p.m., at Birzer Funeral Home in Lyons. Memorial donations to the Heather Radcliffe expense fund may be made in care of the funeral home.
