St. Rose Convenient Care Walk-in Clinic is now offering school sports physical exams from 8 a.m. to 8 p.m. on Mondays and Fridays. The service will be available throughout the summer.

No appointment is necessary but a parent or guardian must accompany the child to the clinic, St. Rose Site Manager Zena Jacobs noted.

“The adults will be required to sign the consent form,” Jacobs explained. “We ask them to bring the school sports physical form to our walk-in clinic with their portion completed.

“We also need the child’s immunization record,” Jacobs said, adding that a school sports physical is not a well-child check.

The $35 fee will be payable at the time of service and will not be sent to an insurance company. There is a $15 fee for each additional form that is completed.

“St. Rose wants to accommodate families’ busy schedules by providing this convenient option for school sports physicals,” Jacobs said. “Parents are encouraged to take advantage of this opportunity.”

Ed Habash, PA-C, and Maggie Myers, MSN, APRN, FNP-BC, are Convenient Care Walk-in Clinic health-care providers. The clinic’s regular hours are 8 a.m. to 8 p.m. Monday through Friday; 9 a.m. to 4:30 p.m. Saturday; and noon to 5 p.m. Sunday.
