Community health needs assessment feedback request - St. Rose Health Center

Over the next three months, St. Rose Health Center will be preparing a 2018 Community Health Needs Assessment (CHNA). The goal of this community assessment is to understand current progress in addressing community health needs cited in previous CHNA reports and to collect up-to-date health opinions and perceptions.
To accomplish this work, a short online survey has been developed: https://www.surveymonkey.com/r/BartonCHNA3
All community residents are encouraged to complete the 2018 CHNA Wave #3 online survey by Friday, Feb. 23. Participants can also find the CHNA feedback link on the St. Rose Health Center website and Facebook page.
“We hope that our community will assist us to provide valuable input into the future of health-care delivery in Great Bend,” said Zena Jacobs, executive director. “Also, we hope that our residents will attend the upcoming scheduled Town Hall at 11:30 a.m. to 1 p.m. on Thursday March 8 from at Holiday Inn Express and Suites 3821 10th Street.”
Vince Vandehaar MBA (VVV Consultants LLC, an independent research firm from Olathe), has been retained to conduct this community research.
